    Perhaps a strange question:

Does there exist a Today screen app that displays a customized list of *large* shortcuts so I can launch apps without using the stylus, i.e. with a finger?

Following application works very well. You can set the icon size to whatever you choose. Typical is 16 bit size, but I just tried 48 bit size and it may be what you are looking for.

I use a couple of programs in conjunction with one another, with the intent of accessing and closing all my applications without the use of a stylus.
    1. My favorite (freeware) Today Screen launcher is tdLaunch:

    2. My favorite launcher program is Conduit?s Pocket Launcher. This is a desk top launcher with a tabbed interface. I have it mapped to a hardware button so I can access all my applications without the use of a stylus:

    3. The Today Screen Icon is a little .exe that lets me go back to the Today screen (so you can create shortcuts to the today screen like a shortcut to any other application). It?s freeware and can be found here:

    4. I also have a Task Closer mapped to a hardware button called ZapIt, which closes any running application with the push of a button (I use this in conjunction with Pelmar?s WisBar, which resides on the Task Bar). ZapIt is also freeware and can be downloaded here:

You may also want to check out Snoopsofts?s Dashboard, which is a customizable viewer for your basic Personal Information Management info and provides built-in views for Agenda, Mail, Tasks and a Program Launcher:

Dashboard offers three ways to display icons; small (which I use to have lots and lots), small with names (useless once you know the icons), and large, like this:

I spend every waking moment trying to keep my fingers off the screen.

Then maybe it's time for a WriteShield? Get 'em all dirty, greasy even, then a quick wipe or rub with a shirt sleeve and they're perfect again. Fine screen protectors.
